Forms!

There are two sets of forms you might want to use with Gambler's Help TrakCare. These are:

• Data capture forms
• System support forms.

Data capture forms

Often, it isn't practicable to enter client information immediately. This set of forms enables you to record on paper data that you can later enter into Gambler's Help TrakCare. Each form in the set emulates a screen, listing each field in order and including the options for each list of values.

Support forms

There are three support forms. Use these to:

1. Report a duplicate client registration
2. Request a change to your user group to add. change or cancel a person's access to GH TrakCare or DeepSee
3. Report a system fault for follow up and resolution.

In general, requests to resolve duplicate registrations or change user access will need to be signed off by the program manager or team leader at the site where you work, but any user can report a system fault.

NOTE! What looks like a system fault can often be a minor problem that is easily fixed . If you're not sure about the problem, ask your local super-user or give Ben a call on 86841904.
